Passport issuance to minor under the age of 16 now requires the personal appearance of the child before the Consular Officer. Both parents are also requested to appear in person. However in case only one parent is applying, the non-applying parent will need to fill out DS 3053 (Statement of Consent) sign and have it notorized. The Consular Officer can notorize this document free of charge for American citizens.
Passport issuance is centralized in the States and applications are transmitted electronically to Portsmouth, New Hampshire.
