Photo Requirements

1. Full-face photo taken within the last six (6) months
   
   
2. A "full-face" photo is one in which the applicant is facing the camera directly
   
   
3. The applicant should not be looking down or to either side, and the face should cover 50 percent of the area of the photo
   
   
4. The head of the applicant, including the face and hair, should be shown.
   
   
5. The ears should be exposed
   
   
6. Photos with sunglasses, facemasks or veils are not acceptable
   
   
7. The applicant must be clearly identifiable from the photograph
   
   
8. Photos of military, airline or other personnel wearing hats are not acceptable
   
   
9. Photos of applicants wearing tribal or other headgear, not specifically religious in nature, are not acceptable
   
10. Photograph should measure 1 1/2 to 2 inches square.
    
    
11. Photograph must be stapled to the application form.
    
12. Photograph should be in color with a white background
